Many bid lists require a COI naming the requesting party. Send the exact certificate holder language and additional-insured requirements your contract calls for so the certificate is issued correctly the first time.

It depends entirely on your process. How far ahead of the bid date should the invitation come in?
Earlier is better, but late is still workable. The constraint on most equipment bids is manufacturer lead time and approved-equal review, not the quote itself. If the bid closes soon, note the date in the form and call (559) 395-5500 directly so the request gets prioritized rather than queued.
